Please read the following information if you are interested in participating in athletics at Holmes for the 2022-23 school year:

Mr. Olschanski (Athletic Director) will be sending out an email for Volleyball and Cross Country on August 22 in the afternoon, the last day to sign up will be August 31. A separate link for each sport will be provided for sign ups.

All 7th and 8th grade students must have a signed, valid LPS Sport Physical form Download LPS Sport Physical form on file with the athletic director no later than the tryout date for the sport they are trying out for.  

Please print both pages and read the entire form, and be sure to take this form to the doctor to fill out medical information and sign and date.

The form WILL NOT be valid without a doctors signature and appropriate date.  

To be valid for the 2022-2023 school year, a sport physical must be dated AFTER APRIL 15, 2022.
