PLEASE ONLY FILL OUT THIS FORM IF YOU WOULD LIKE TO OPT YOUR CHILD OUT OF BEING IN PHOTOS AND VIDEOS (see examples below of how LPS uses photos and videos).
Livonia Public Schools uses photography and video recordings of students in print, on social media and online communications to share with our community the great things happening in Livonia Public Schools. In addition, photos and videos are used to enhance the quality of our marketing and informational materials.
The district will not share student names in these promotional uses.
You have the right to opt-out of your child being included in photos and videos by completing this form. If you have more than one child in the district, please fill out a form for each.
If the "Media Opt-Out form" is NOT completed, participation in LPS activities implies permission for the use of images taken at school / district events.
